Evidence of insurability is the proof of good health that an insurer requires before approving underwritten coverage. EOI typically consists of a completed medical questionnaire, but in some cases, the insurer may request additional evidence such as a paramedical exam, blood tests, urine samples, an attending physician's statement, or medical records from your doctor. The level of evidence required usually depends on the type of coverage, the coverage amount, and your age — older applicants and those seeking higher coverage amounts are more likely to need additional evidence. EOI is not required for guaranteed-issue or conversion plans. The EOI process can take one to four weeks, depending on whether additional medical information is needed.
